By "hooked up right" you mean the black connector on each end of the speaker wire goes to the black terminal and red goes to red? And you've verified that the banana plugs or spade lugs are connected properly to the wire (someone could have reversed the plugs when they put them on the wire if these are terminated by hand). Or if you're using bare wire, then you have the same wire going to positive and negative on each end, yeah?

It's possible that the phase error is caused by speaker positioning, or blockage as Don said. It's also possible that one of your speakers is actually cross-wired internally. Does it tell you specifically which channel is out of phase or is it just a general phase error?
